Waiver and Release of Liability

In consideration of the services provided by OC Salsa Dance Studio, the undersigned, on behalf of myself, my minor child, and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby acknowledge, appreciate, and agree to the following:

1. Assumption of Risk: I acknowledge that dance classes, including but not limited to salsa and bachata, involve physical activity that may carry certain inherent risks, including but not limited to falls, sprains, strains, and other injuries. I voluntarily assume full responsibility for any risks of injury or loss, including death, that my child may sustain as a result of participating in these classes.

2. Medical Authorization: I certify that my child is physically fit, has sufficiently prepared for participation in the dance classes, and has not been advised to refrain from physical activity by a qualified medical professional. I understand that OC Salsa Dance Studio may not have medical personnel on site and that in the event of a medical emergency, 911 will be called, and my child will be transported to the nearest hospital.

3. Release of Liability: I, on behalf of my minor child, myself, and our respective heirs, assigns, administrators, personal representatives, and next of kin, hereby release, discharge, and hold harmless OC Salsa Dance Studio, its owners, directors, instructors, employees, volunteers, and representatives from any and all liability, claims, demands, or causes of action that may arise from my child’s participation in the dance classes, including but not limited to personal injury, property damage, or wrongful death, whether caused by the negligence of OC Salsa Dance Studio or otherwise.

4. Indemnification: I agree to indemnify, defend, and hold harmless OC Salsa Dance Studio, its owners, directors, instructors, employees, volunteers, and representatives from any and all claims, demands, actions, and expenses (including attorney fees) arising out of my child’s participation in the dance classes.

5. Photography and Video Release: I grant permission for OC Salsa Dance Studio to use photographs and/or video recordings of my child taken during dance classes for promotional purposes, including but not limited to the studio’s website, social media pages, and marketing materials. I understand that no compensation will be provided for such use.

6. Agreement to Follow Rules: I agree that my child will adhere to all studio rules, guidelines, and instructions provided by OC Salsa Dance Studio and its instructors. I understand that failure to follow these rules may result in my child’s dismissal from the program.

7. Severability: If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

Parent/Guardian Acknowledgment

I have read and fully understand this Liability Waiver and Release Form. I am aware that by registering to today’s class and all following classes ad events at and by Oc salsa , I am waiving certain legal rights, including the right to sue, and I agree to its terms voluntarily.
